Bayer did not invent PDE5 blockade. Sildenafil had already opened the market. The Wuppertal chemistry group built a close analogue, co-marketed in the US with GSK as Levitra in 2003, the same year tadalafil arrived. Fast-follower is not an insult. Fasted Tmax is roughly 30 to 60 minutes for the swallowed tablet. Half-life runs about four to five hours. The working window is several hours, not tadalafil's day-and-a-half. For a man who wants a drug that is in and out on a known clock, that is a feature.

Quinidine and procainamide (Class IA) and amiodarone and sotalol (Class III) are the named avoid pair with vardenafil. Congenital long QT is the other named avoid. If those are on the chart, I write sildenafil or tadalafil after I have still cleared nitrates, not a 'small' 5 mg Levitra as a compromise.

Men 65 and older, and men with moderate hepatic impairment, often start at 5 mg. Severe hepatic impairment is a reason to avoid. Renal impairment matters less than it does for some drugs, because the kidney is a smaller exit, but I still read the list.
Anyone already on a moderate CYP3A4 inhibitor starts at or below 5 mg. Anyone on a strong inhibitor is in the 2.5 mg / long-gap room or off the drug. That is label, not timidity.
Women and children are not an ED population for this tablet. The population question that is special here is electrical: a personal or family arrhythmia story, a QT-active list, or a congenital long-QT diagnosis. He is on amiodarone. Is a tiny vardenafil dose all right?
I do not write vardenafil next to amiodarone, sotalol, quinidine, or procainamide, and I do not call 5 mg a compromise. The label's avoid list exists because QT effects add. If nitrates are also off his list, sildenafil or tadalafil may still be discussable with the clinician who owns his rhythm. A tiny Levitra is not a loophole.
He keeps a nitrate tablet 'just in case'. Does that close Levitra?
Yes. Any nitrate in any form - including a tablet in a drawer he swears he rarely uses - closes the whole PDE5 class.
