The price ping arrives before the history. People type cheap Neurontin 600 mg, generic gabapentin, order online, and expect a brand story. The tablet they mean is a 1998 scored film-coat, not a 1970s sketch.

A US Neurontin 600 mg tablet is white, elliptical, scored, and stamped NT 16. The Food and Drug Administration cleared that strength on 9 October 1998 under NDA 020882, five years after the first capsule licence. Capsules stay at 100, 300, and 400 mg. The 600 and 800 mg units are tablets. If a cart shows a 600 mg capsule, the listing is already noise.

Generic gabapentin 600 mg is usually cheap because the patent died years ago. Cheap is not a clinical clearance. The scored 600 mg tablet can be split, and unused halves should be taken as the next dose or discarded after 28 days, per the current label. The GABA name was the first false ping

Chemists in the 1970s wanted a GABA analogue that could cross into brain. Plain GABA does not. They fused a cyclohexane ring onto the amino acid and named the product after the parent they hoped it would impersonate. The bet failed in a useful way. Gabapentin does not bind GABA-A or GABA-B, does not become GABA, and does not explain its effect by blocking GABA breakdown.

Years later the lock was named: the alpha-2-delta subunit on voltage-gated calcium channels at nerve terminals. Occupy that subunit and fewer channels reach the membrane. Less calcium enters when the terminal fires. Less glutamate and substance P leave. Over-firing quiets. That is a calcium-channel story wearing a GABA badge. Residents who reason from the badge write the wrong consult.

The name stuck because the drug was already selling when the mechanism caught up. Nobody renames a marketed anticonvulsant for the sake of honesty. So the first flag on any Neurontin file is still the same: ignore the etymology, keep the subunit.

Ann Arbor, Parke-Davis, and a quiet European start

Parke-Davis, then the drug arm of Warner-Lambert, ran the gabapentin bench in Ann Arbor, Michigan. Later FDA letters still list Plymouth Road. That address is not folklore.

Add-on anticonvulsants rarely launch with a parade. Neurontin reached European patients before the US licence, and the early forecast was modest: bolt it onto an existing seizure drug when partial seizures still broke through. Nobody in Ann Arbor was drawing a blockbuster poster. The molecule looked like a supporting actor.

That quiet start matters because it explains the later shock. A drug designed as adjunctive therapy does not need a huge monotherapy story. It needs a clean interaction profile and a tolerable climb. Gabapentin is not metabolised by the liver and does not induce or inhibit the CYP enzymes other anticonvulsants live on. That cleanliness is why epileptologists could add it without rebuilding the whole stack. It is also why, later, pain clinics could add it without a drug-interaction lecture. Clean is not the same as proven for every complaint.

December 1993: an add-on licence, not a solo career

The US Food and Drug Administration approved Neurontin in December 1993 as adjunctive therapy for partial (focal) seizures in adults. Adjunctive is the load-bearing word. It was cleared to sit beside another antiseizure drug, not to replace one. The Department of Justice later repeated that narrow first licence when it described the marketing case. A supporting actor got a supporting licence.

The current label also covers adjunctive partial-onset seizures, with or without secondary generalisation, in patients 3 years and older. That paediatric line is a later widening, not the 1993 opening. Treating the first approval as a general epilepsy stamp is how the noise starts. Monotherapy for new-onset epilepsy was never the original ping.

Capsules launched first. The 600 mg film-coated tablet arrived on 9 October 1998 with an 800 mg sibling. For epilepsy in patients 12 and older, the labelled start is 300 mg three times daily, and the climb may reach 600 mg three times daily - 1800 mg a day. For postherpetic neuralgia the usual target is also 1800 mg a day, which is 600 mg three times daily after a slower first-week ladder. So 600 mg is both a tablet and, later, a single dose on a three-times-daily grid. Those are not the same instruction. Mixing them is how a cheap start becomes a hard first day.

Pfizer inherits the brand, then a pain flag lands

Pfizer bought Warner-Lambert in June 2000. The prize most analysts named was atorvastatin. Neurontin rode along. By then a large share of Neurontin prescriptions already sat outside the 1993 seizure add-on. Pfizer inherited the revenue and the liability in the same closing binder.

In 2002 the label gained postherpetic neuralgia in adults - the burning, stabbing after-pain that can outlast a shingles rash. That was the first approved pain signal, and it was a real one. A separate product, gabapentin enacarbil, arrived in 2011 as Horizant for moderate-to-severe primary restless legs, with a PHN add in 2012. The 2004 flag: promotion, not a poison verdict

Doctors may prescribe off-label. A company may not market off-label. That line is the whole case. Crossing it is a promotion crime, not a proof that the molecule fails its real uses.

David Franklin, a Parke-Davis medical liaison, filed a False Claims Act suit in 1996. He described paid talks, ghostwritten papers, and a push into bipolar disorder, migraine, general pain, and other uses the 1993 licence never named. On 13 May 2004 the Department of Justice announced that Warner-Lambert would plead guilty and pay more than $430 million in criminal and civil resolutions. The criminal fine alone was $240 million. Franklin's relator share was reported at about $24.6 million.

The government's own recap is still the cleanest primary: the 13 May 2004 DOJ release. It restates the December 1993 add-on limit in plain English. Court papers from that era also put a hard number on the drift: more than three-quarters of Neurontin prescriptions in 2000 were already written for unapproved uses. That is a volume signal, not an efficacy signal.

Keep the two files separate. The scandal is how the drug was sold. The pharmacology of the 600 mg tablet is how the drug works when the indication is real.
